How to Enroll

Interested in enrolling your child at one of our Clubhouses? Click the link below to download and fill out an application. Once completed, visit your preferred Clubhouse location to submit the application and learn about additional requirements.

Please download, fill out, and email the above membership application to info@madisonsquare.org. In the body of the email please include your name, address, and the names, ages, and dates of birth of the children you are interested in enrolling and someone will be in touch with next steps.
